Google says local results are based mainly on relevance, distance and popularity. In a region this large, distance does most of the sorting. A search for a pediatrician in Alpharetta and the same search in East Point, thirty miles apart, return completely different practices.
People search by where they live. Inside the Perimeter they name neighborhoods: Midtown, Buckhead, Grant Park, Kirkwood, West End. Outside it they name cities and sometimes counties: Marietta, Roswell, Lawrenceville, Decatur, Cobb, Gwinnett. Each is a separate contest with its own competitors.
